Komunikasi dan Kolaborasi Perawat Untuk Menghasilkan Continuity of Care Pada Pasien Rawat Jalan: Systematic Review

Abstract


Continuity of Care sangat berkaitan dengan efek perawatan pasien, apabila semakin bertambahnya usia populasi dengan tantangan gaya hidup dan penyakit kronis yang signifikan. Hal ini menyebabkan jumlah kunjungan pasien rawat jalan akan semakin tinggi, dan memerlukan perawatan yang berkelanjutan. Perawat rawat jalan sangat berperan penting dalam menyediakan Continuity of Care bagi pasien. Tujuan systematic review ini untuk melakukan eksplorasi peran perawat dalam hal komunikasi dan kolaborasi dalam menghasilkan perawatan berkelanjutan/COC pada pasien di rawat jalan. Penelitian ini menggunakan metode systematic review, dan menggunakan database berbasis ilmiah antara lain: ProQuest, PubMed, ScienceDirect, Sage Journals, EBSCO Host dan Scopus. Hasil pencarian didapatkan 90 jurnal setelah dianalisis lebih lanjut dan 6 artikel yang akan dianalisa. Beberapa topik artikel yang akan dianalisa adalah penggunaan komunikasi dan kolaborasi oleh perawat untuk pasien rawat jalan dalam menghasilkan continuity of care. Hasil literatur ini didapatkan bahwa Continuity of Care bagi pasien rawat jalan dapat dihasilkan apabila perawat melakukan komunikasi dan kolaborasi yang baik antara perawat dan pasien, keluarga, pengasuh, dan profesi kesehatan lainnya. Komunikasi dan kolaborasi membuat semua anggota tim pelayanan kesehatan sebagai mitra dalam proses perawatan yang berfokus pada kebutuhan pasien dalam menghasilkan perawatan berkelanjutan.
